Doctors are no longer allowed to conduct off-site school physicals due to liability. The following is a list of Doctors and/or facilities that have agreed to provide student athletes with a physical for $50.00. Call to make an appointment. Take the county physical form with you and be prepared to complete the proper patient forms in the doctor's office. Payment wil be collected when the athlete checks in for his/her appointment. Cash, check, Visa, and MasterCard will be accepted. You will be provided with a receipt should you choose to turn it in to your insurance company for re-imbursement.
